Ability to transform link desciptions in Muse

I've made it so that link descriptions can be transformed in the same way that links themselves can be transformed.

To make use of this, customize muse-publish-desc-transforms. For example, I use both `muse-wiki-publish-pretty-titles' and `muse-wiki-publish-pretty-interwiki' for this option to publish my site. Combined, this changes "InterWiki#LinkedPage" to "Inter Wiki: Linked Page".
